Family SHADE Youth Transition Services Workshop and November Membership Meeting – November 4th and 5th!!

The Family SHADE November Membership Meeting will be held in two sessions over two consecutive days.

The first session of the Family SHADE November Membership Meeting will be a workshop on November 4, 2020 from 10:00am-12:00pm. The Family SHADE Youth Transition Services Workshop will be held virtually via a Zoom webinar platform. 

The transition from youth to adulthood is an important time for every young person, including youth with special healthcare needs. As adolescents with a disability or chronic health condition are moving into adulthood, there are often concerns about planning for the future including options for postsecondary education, careers, health care, financial benefits, legal matters pertaining to guardianship and other areas.  

The continuity of services for youth transitioning across both child and adult service systems is critical.  This workshop will cover information regarding youth transition services available in Delaware in the following areas:

  • Education
  • Employment
  • Healthcare
  • Legal Matters

The second session of the Family SHADE November Membership Meeting will be held the next day, on November 5, 2020 from 10am-12pm. This will be a virtual event that will begin with an information session provided by Unite Delaware, followed by Family SHADE activities, information and other business. 

Unite Delaware is a statewide coordinated network that connects health and social service organizations through a shared technology platform, Unite Us, to send and receive trackable referrals. Unite Delaware is sponsored by Christiana Care.

Community-based organizations that participate in the Unite Delaware network are connected through the Unite Us platform, which allows them to expand their organizational impact by easily connecting their clients to a wealth of services in less time than before, track their clients’ journey across the community, and report on tangible outcomes across a range of services and sectors. It is completely free for any community-based organization to participate in the network.
