It can be helpful for children with special healthcare needs to be part of a medical home. What is a medical home? A medical home provides care that is accessible, continuous, comprehensive, patient- and family-centered, coordinated, compassionate and culturally effective. The Delaware Center for Health Innovation is a non-profit organization, working to guide the implementation of Delaware's State Health Care Innovation plan in conjunction with multi-stakeholders who are committed to achieving sustainable health care transformation, resulting in better health outcomes; improved health care quality and enhanced provider experience.  Board meetings are monthly and public comments are welcome.  For dates and location of meetings contact DCHI.
